All clients please download and complete a Credit Card Form to keep on file for unpaid charges.
Sexual Addiction Assessment
Please complete the SA assessment to determine if you might be struggling with a sexual addiction.
Adult and Teen Assessment
Please answer each question to the best of your ability. If you are completing this assessment for your minor child, please give it to him/her to add to in privacy as you have completed your information.
